I consent to the data I have entered into this form, being collected, stored, and used in relation to the booking of an appointment with E. Dando Holistcs.
E. Dando Holistics may contact you prior to your appointment, by email or telephone, if based on your above responses there is a need to discuss your suitability for treatment.
Upon submitting this completed form you will then be redirected to the ‘Fresha’ booking platform. This is the platform E. Dando Holistics uses to facilitate online bookings. This facility allows you to choose a clinic location, appointment/treatment type, and a date/time for an appointment that is convenient for you. During the booking process Fresha will request card details to be entered in order to secure your appointment. No deposit is required up front to confirm your booking. Card details are only ever used in the event of a late cancellation (less than 48 hours' notice given), or in the event of a no-show. In these scenarios Fresha charges a late cancellation fee (50% of the appointment amount) or a no-show fee (100% of the appointment amount), as appropriate, to the card on file.
Rescheduling of appointments is permitted where there is more than 48 hours' notice given, and can be made without incurring any fees.
